Currently, Dr. Pal serves as an Assistant Professor in the Department of Infectious Diseases at the School of Tropical Medicine, where he is involved in clinical care, research, and training of medical professionals. A 26-year-old male IT professional presented with a history of recurrent pneumonia for 3 months, accompanied by loss of appetite, weight loss, chronic diarrhoea, and extreme weakness. On evaluation, he tested HIV positive, with a CD4 count of only 7 cells/Β΅L, indicating advanced immunosuppression.
Further investigations, including chest and abdominal CT scans, were normal. However, colonoscopy revealed CMV colitis (Cytomegalovirus infection of the colon), a common opportunistic infection in severely immunocompromised patients. He was started on Ganciclovir for CMV colitis and subsequently initiated on Antiretroviral Therapy (ART) under specialist supervision.
Within weeks of starting ART, his diarrhoea subsided, appetite improved, and he began to gain weight gradually. Over the following months, his health continued to improve steadily. After 6 months of treatment, his CD4 count increased to 171 cells/Β΅L β a remarkable sign of immune recovery.
He has now regained his strength, resumed normal activities, and returned to work in Hyderabad, continuing ART with regular follow-up.

A 37-year-old hotel owner from Kolkata presented with prolonged fever, significant weight loss, and loss of appetite. On evaluation, he tested HIV positive. Further examination revealed enlarged neck lymph nodes.
A biopsy of the neck glands confirmed Tuberculosis (TB). Soon after, he also tested sputum positive for TB. Additionally, he developed CMV (Cytomegalovirus) retinitis, a viral eye infection that can occur when the immune system is very weak.
He was started on anti-TB medicines, and although his lymph nodes initially enlarged (a known immune reaction), they later reduced in size with continued therapy. After 10 days, HIV treatment (ART) was started under specialist supervision.
With regular treatment and close follow-up, the patient showed steady improvement. His appetite and strength returned, and his eye infection completely healed. Over the next few months, his CD4 count increased from 74 to 125 cells/Β΅L, showing significant immune recovery. He continues ART and TB therapy with good tolerance and excellent response.
